Sports Screening Program

Licking County students entering grades 7 through 12 who participate in sports, marching band, and C-TEC’s Criminal Justice and Physical Therapy programs are invited to attend the LMHS Sports Screening Program at no cost. Dates for the 2024 program are available from
May 2 to May 30.

Registration is required in advance and limited to the first 90 participants each night of the program. After registering, you will be redirected to a packet of forms that must be printed out and completed prior to your arrival at the Sports Screening Program. Click here to register.

Additionally, any student who requires a sports physical must provide the completed Ohio High School Athletic Association (OHSAA) pre-participation physical evaluation (PPE) form or your school’s equivalent – signed by a parent or guardian – at check in.
