2. A Release of Medical Information Form in a PDF format can be downloaded and sent to us when completed via FAX at (740) 348-4129, United States Postal Service, or bring it in person. You will need Adobe Acrobat Reader to view PDF files. If you do not have it, you may download the Adobe Reader from the Adobe Web site. Click here to download.

Please note: Requests for information require the patient's signature and the form needs to be completed in full.
