Community Case Management is a collaborative process that assesses, plans, implements, coordinates, monitors and evaluates the needs and services that may be necessary to meet your needs after hospital discharge. Licking Memorial Hospital provides this service without cost to the patient and family.
- Community case managers include registered nurses who work with your physician to assist you in meeting your health care goals by:
- Coordinating needed care and services provided by health care professionals and the many agencies that provide community services.
- Communicating with others in the Health Systems, serving as an information channel to you and on your behalf.
- Educating you and your family about how to prevent complications and how to promote healthy living.
- Supporting and empowering you to make appropriate and responsible health care decisions, to gain increased competence and independence when it comes to managing your own health.
- In addition to the registered nurses, the community case management team includes:
- Dietitians
- Fitness coordinator
- Pharmacist
- Community case managers have multiple roles, which include being a consultant, advocate and motivator.
