Sports Screening Program

Licking County students, entering grades 7 through 12, who participate in sports, marching band, and C-TEC’s Criminal Justice and Physical Therapy programs are invited to attend the LMHS Sports Screening Program at no cost. Dates for the 2026 program are available from May 4 to May 27.

Registration is required in advance and limited to the first 90 participants each night of the program. To register, click the link below. After registering, you will be redirected to a student-specific packet of forms that must be printed out and completed prior to your arrival at the Sports Screening Program.

Registration opens on April 1.

Additionally, any student who requires a sports physical must provide the completed Ohio High School Athletic Association (OHSAA) pre-participation physical evaluation (PPE) form or your school’s equivalent – signed by a parent or guardian – at check in.
