MyChart Paperless Billing Opt-out
By default, MyChart users are automatically enrolled in paperless billing upon signup. Billing statements are emailed to users from MyChart instead of receiving a paper statement in the mail. If you would prefer to receive a paper statement for your billing accounts, follow the steps below:
- Log into MyChart via the mobile app or website (MyChart.LMHealth.org).
- Select Menu.
- Scroll to the Billing section and select Billing Summary.
- Click the “cancel paperless billing” hyperlink at the bottom of the page.
- Follow the prompts, then click the Cancel Paperless button to confirm the change to paper statements.
To receive assistance with making this change, please call the LMHS Information Systems Service Desk at (220) 564-4030, between 8:00 a.m. and 8:00 p.m., Monday through Friday. For questions about your bill, contact Licking Memorial Patient Financial Services at (220) 564-1500, between 7:30 a.m. and 5:00 p.m., Monday through Friday.
Previous Patient Portals (MyLMHealth, Physician Office Patient Portal)
Medical information including current conditions and problems, allergies and medications has been transferred from the previous LMHS patient portals to your MyChart account. Other information, such as lab results, pathology results, departmental reports and radiology within the last three years, also have been transferred to MyChart. Your older historical medical record information will continue to be retained at LMHS even though it may not be accessible from the new portal.
To obtain a copy of your medical record, please download this Release of Medical Information Form The completed form can be sent to us via FAX at (220) 564-7717 or mailed to Licking Memorial Hospital, 1320 West Main Street, Newark, Ohio 43055. If you have any questions about your medical record, please contact the Health Information Department at (220) 564-4122.
Please note: Requests for information require the patient's signature and the form needs to be completed in full.
