How are nutrition services or diabetes classes obtained?
Your physician may refer you to the program, or you can schedule your appointment through LMH Central Scheduling at (220) 564-4722.  You must have your physician provide an order for the classes and nutritional counseling. 

When are the dietitians and diabetes classes available?
The initial appointment with the dietitian is made by appointment.  It usually takes approximately 1-1½ hours for the nutrition assessment and all required education.  During that visit, you will also be provided with a personalized meal plan.  The classes are offered in a series of two 3-hour sessions, the first and second Wednesday of each month, from 9:00 a.m. to 12:00 Noon.  May and October classes are held the first and second Wednesday of each month, from 5:30 to 8:30 p.m.  For general information about the Diabetes Self-Management classes, please call (220) 564-4915.

What if I need to cancel or reschedule my appointment with the dietitian?
If you have questions or if you need to cancel or reschedule your appointment or classes, you should call LMH Central Scheduling at (220) 564-4722. 

Is there financial assistance available for the nutrition services and diabetes classes?
Most insurance companies will cover the cost of nutrition services and diabetes classes as long as they were ordered by a physician.  For those who do not have insurance coverage, LMH has financial assistance available upon request by calling the Billing Office at (220) 564-4500 for an application.
