Careful monitoring of some blood-thinning medications – such as Warfarin (Coumadin), Lovenox or Heparin – is necessary because of possible serious risks associated with certain health conditions, such as stroke, atrial fibrillation and deep vein thrombosis, as well as the medication, itself. The Licking Memorial Hospital Medication Therapy Clinic is a service directed by a physician and staffed by clinical pharmacists and registered nurses with specific knowledge of these types of anticoagulation therapy. Laboratory studies are generally performed on-site in the clinic using a finger stick. Patients should report any active bleeding episodes or severe bruising. Expected length of therapy is determined by your primary care physician and is made on an individual basis.
Diabetes management is provided for patients with a primary diagnosis of Type 1 diabetes or Type 2 diabetes. Clinical pharmacists, nurses and dietitians with specific knowledge and training in diabetes work together to help patients reach personal goals for better health. Our purpose is to create individual treatment plans that allow patients to understand more about the dosing of their medications. We offer a variety of ways to help monitor and report glucose levels. We also help patients learn the signs and symptoms to reduce the risk of diabetes-related complications.
Heart Failure
Heart Failure management is a service directed by a physician and staffed by clinical pharmacists and nurses with specific knowledge of identifying and managing symptoms as well as adjusting heart medications. We work closely with cardiologists and primary care physicians to ensure that appropriate medications are both tolerated and at the most effective doses. This helps to minimize hospitalizations and reduce the risk of cardiac events. Dietary education and support also is provided.
Gastroenterology
Gastroenterology medication management is provided for therapies used to treat gastrointestinal and liver diseases, including Crohn's disease, ulcerative colitis, and hepatitis C. Our team provides comprehensive medication education and training on proper administration techniques to help patients understand their treatment plans and achieve the best possible outcomes. We also help guide patients through important follow-up care, including laboratory monitoring, scheduled appointments, management of medication side effects, and assessment of treatment effectiveness. We work with gastroenterologists and the LMH Specialty Pharmacy to assist patients with obtaining prior authorization approvals and identifying financial assistance programs for eligible specialty medications.
